Individual Plan – $300 per Year
Includes:
- Two professional cleanings (adult prophylaxis)
- Two comprehensive exams
- Routine digital X-rays as needed
- 20% discount on most additional treatment*
Couple Plan – $500 per Year
Includes all the benefits of the Individual Plan for two family members.
Family Plan – $600 per Year
Includes all the benefits of the Individual Plan for a family of four.
- Add additional children for $100 per child
Periodontal Maintenance Plan – $450 per Year
Designed for patients who require periodontal maintenance following gum disease treatment.
Includes:
- Three periodontal maintenance cleanings per year
- Routine exams
- Necessary X-rays
- 20% discount on most additional treatment*
